Who this policy applies to

This Privacy Policy applies to Signature Dental and to personal information collected through this website, by phone, by email, through online booking, in person at the clinic, and during the usual administration of dental care.

Personal information means information or an opinion that identifies you or could reasonably identify you. Health information is sensitive information under Australian privacy law and may include your dental health, appointment details, treatment records, billing information and related clinical notes.

Information we may collect

The information collected depends on how you interact with the clinic and what care you ask about or receive.

  • Your name, date of birth, contact details and emergency contact details.
  • Appointment requests, enquiry details and communication preferences.
  • Dental and medical history, symptoms, diagnoses, treatment notes, X-rays, scans, photographs where clinically needed, consent forms and referral details.
  • Medicare, private health fund, HICAPS, payment and billing information where relevant.
  • Website information such as pages visited, device details, browser type and general analytics data.

How information is collected

Most information is collected directly from you, for example when you complete a form, book online, speak with the team, email the clinic or attend an appointment.

Information may also be collected from a parent, guardian, carer, referring practitioner, insurer, health fund, payment provider, government service or another health professional where it is lawful and relevant to your care or administration.

How information is used

Signature Dental uses personal information for ordinary clinical and administrative purposes connected with dental care.

  • To respond to enquiries and manage appointments.
  • To provide dental assessment, treatment planning, treatment, referrals and follow-up care.
  • To process accounts, health fund claims and payment arrangements.
  • To maintain clinical records and meet legal, professional and insurance obligations.
  • To improve clinic systems, website performance and patient communication.
  • To send practice updates or reminders where permitted, with an option to opt out where required.

When information may be shared

Personal information is shared only where reasonably necessary for care, administration, law, safety or another permitted purpose.

  • With dentists, dental assistants and administrative team members involved in your care.
  • With external dental practitioners, laboratories, radiology providers, medical practitioners, pharmacists or other health providers where relevant.
  • With health funds, Medicare, payment providers, insurers, professional advisers or technology providers where needed for administration.
  • Where required or authorised by law, court order, regulator request, public health requirement or a serious threat to life, health or safety.
  • With your consent, or where you would reasonably expect disclosure for a purpose related to your care.

Storage and security

Signature Dental takes reasonable steps to protect personal information from misuse, interference, loss, unauthorised access, modification and disclosure.

Information may be stored in clinical software, secure practice systems, email systems, booking systems, payment systems, paper records and carefully managed backups. Some technology providers may store or process limited information outside Australia depending on their systems.

Retention and eligible data breaches

Clinical and administrative records are kept for as long as required for dental care, legal, professional, insurance and business record-keeping purposes. When information is no longer required, the clinic takes reasonable steps to destroy it or de-identify it where appropriate and lawful.

If a data breach is likely to result in serious harm and notification is required under Australian law, Signature Dental will take the steps required by law. This may include notifying affected people and the Office of the Australian Information Commissioner.

Access and correction

You can ask for access to personal information Signature Dental holds about you. You can also ask for information to be corrected if you believe it is inaccurate, incomplete or out of date.

Some requests may need to be verified, limited or refused where the law allows or requires it, for example where another person's privacy is involved or where releasing information would create a serious risk. The clinic will explain the reason where it can.

Questions and complaints

If you have a question or complaint about privacy, contact Signature Dental first so the team can review the issue and respond. You can contact the clinic by email at info@signaturedental.com.au or by phone on (08) 9474 4750.

The clinic will consider privacy complaints and respond within a reasonable time. If you are not satisfied with the response, you can contact the Office of the Australian Information Commissioner or another relevant regulator.
